Regulation Title: SOCIAL WORKER RESPONSIBILITIES

Regulation Description: The qualified social worker is responsible for conducting psychosocial evaluations, participating in team review of patient progress and recommending changes in treatment based on the patient's current psychosocial needs, providing casework and groupwork services to patients and their families in dealing with the special problems associated with ESRD, and identifying community social agencies and other resources and assisting patients and families to utilize them.

Surveyor Findings:


Based on medical record review and staff interview, the facility failed to provide adequate social services in 5 (#1, #2, #3, #4 and #5) of five medical records reviewed. This failure created the potential for the psychosocial needs of patients to go unmet. The findings were:

The medical record for sample patient #1 was reviewed on 12/5/07. The patient was admitted on 3/27/06 for the first hemodialysis treatment. There was a psychosocial evaluation performed five months later on 8/14/06. However, there was no further documentation by a social worker since 1/18/07.

The medical record for sample patient #2 was reviewed on 12/5/07. The patient was admitted on 8/31/07 for the first hemodialysis treatment. There was no evidence in the medical record that a psychosocial evaluation had been performed or any documentation by a social worker.

The medical record for sample patient #3 was reviewed on 12/5/07. The patient was admitted on 6/15/07 for the first hemodialysis treatment. The initial psychosocial assessment was performed five months later on 11/12/07. There was no documentation by a social worker in the intervening months between 6/07 and 11/12/07.

The medical record for sample patient #4 was reviewed on 12/5/07. The patient was admitted on 3/22/06 for the first hemodialysis treatment. There was no documentation in the medical record by social services from 1/07 through 11/12/07. On 11/12/07 a psychosocial evaluation was performed.

The medical record for sample patient #5 was reviewed on 12/5/07. The patient was admitted on 10/20/05 for the first hemodialysis treatment. The most recent psychosocial evaluation was performed on 8/4/06; there was no further documentation by a social work since 1/07.

The social worker (SW) was interviewed on 12/5/07 at 10:30 a.m. The SW stated that s/he had terminated her employment with the facility in either 1/07 or 2/07 and had been rehired in 11/07. During that span of time between 2/07 to 11/07, no social services were performed in the facility. The SW further stated s/he was given a limited amount of time for each patient and sometimes this was difficult because some patients have limited needs and others have very extensive needs.


Facility Plan of Correction:

Upon review of charts:

Patient #1's initial psychosocial evaluation was completed 8/14/2006. Annual evaualtion was completed 12/5/07. see attachments

Patient #2's initial psyschosocial evaluation was completed 11/12/07. see attachment.

Patient #3's initial psyschosocial evaluation was completed 11/12/07. see attachment.

Patient #4's initial physchosocial evaluation was completed 11/12/07. see attachment.

Patient #5's initial psyschosocial evaluation was completed 08/04/2006. An Annual evaualtion will be completed by 01/31/2008. see attachment.

Additional time will be allowed for the Social Worker to review and address the needs of patients. This time will also be used to update all patient records. The exact amount of additional time will be determined at a meeting with the Social Worker.

The Director of Nursing will contact the Social Worker at the beginning of each month to ensure that adequate time has been alloted to meet patient's needs.

The policy of Summit Kidney Centers is that all Long term care plans, physicians Standing Orders, Nursing History and Physical, Treatment options, Psycosocial Assessments, Dietiary Assessments and Consent to dialyze forms are to be monitored by the Director of Nursing. This responsiblity may be delegated to the appropriate personnel.

QA at this time states that all charts will be reviewed quarterly. A data base that monitors and flag dates and requirments has been started in the computer, a designated person will monitor and update files. The results will be reported to the DON and upward to the Medical Director
